The dynamic Health Dialogues, which are delivered on the Health Hero technology platform, are designed to facilitate automated communication between patients and their healthcare providers. The Health Dialogues are sets of questions and answers based on standard practice guidelines and include educational reinforcement and reminders to help patients better manage their own care. Health Hero Network offers Health Dialogues pre-assembled into individual programs, or customers and partners may opt to create new Health Dialogues and programs tailored to their specific needs. Embedded outcomes and measurements are built into the patient-focused Health Dialogues to enhance care provider decision-making.
Current customers use Health Dialogues to manage thousands of patients with chronic conditions including congestive heart failure, chronic obstructive pulmonary disease, diabetes, coronary artery disease, hypertension, depression, cancer care, chronic pain, HIV-positive, hepatitis C, schizophrenia, bi-polar disorder and other diseases across the U.S. and Puerto Rico.
