“The Pfizer award is yet another testimony to the fact that, with the right tools, committed health care organizations like the Veterans Health Administration can leverage their expertise to achieve stunning results in disease management”, said Steve Brown, CEO of Health Hero Network.

Their initiative is meant to improve clinical outcomes, reduce costs, and increase patient satisfaction. The Lake City Rural Home Care Project, a unit of the HCCSL, recently reported 10 million dollars in health care savings across 190 patients. Health Hero Technology was used to care for 115 of the Lake City patients. The 115 patients using the Health Hero® Health Buddy® appliance showed 74% reduction in hospitalizations, 78% reduction in ER/Walk in visits, and a 93% patient satisfaction score.
The Health Hero platform, used by hospitals, medical groups, health plans, disease management, pharmaceutical companies and the Veterans Health Administration, consists of the Health Hero® iCare Desktop™, a web-based patient management tool, and Health Buddy®, an easy-to-use information appliance placed in the patient’s home. Together the web tools and the appliance form an integrated solution that allows for daily personalized communications which helps care managers identify potential problems with patients before they occur, while facilitating behavior modification and education on self-management.
